Correct an NPPES source record
If an NPI name, taxonomy, address, or lifecycle field is wrong in the authoritative record, the enumerated provider or authorized official should update it through the official NPPES process. NPIFolio cannot change a CMS source record.

Email the NPI, the page URL, a concise description of the issue, and—when available—a public source that supports the request. NPIFolio will review whether its displayed record, transformation, or freshness information differs from the approved source.
